The Community Health Improvement Plan (CHIP) is the second prerequisite required of The Public Health Division in order to apply for public health accreditation through the Public Health Accreditation Board (PHAB).
A CHIP is a long-term, systematic effort to address public health focus areas that were identified through the community health assessment activities. No single entity has the ability to improve the public’s health and this plan brings together our partners to coordinate and target resources. A community health improvement plan is critical for developing policies and defining actions to target efforts that promote health. It will define the vision for the health of El Dorado County through a collaborative process and will address the gamut of strengths, weaknesses, challenges, and opportunities that exist in El Dorado County to improve the health of our residents.
